Primary Care Physician

Cares Community Health is seeking a Primary Care Physician to see walk-in, urgent care and pre-scheduled patients. The Physician will work with other staff members including Nurses, Medical Assistants, Laboratory staff, Behavioral Health and Social Workers, as well as with the Dental team. Eight hours a month of Saturday or evening clinics expected. We are seeking a Physician who has a specialty in Internal Medicine, Family Medicine and a desire to work with underserved populations. Site Medical Director

Cares Community Health has an immediate opening for a full time Site Medical Director. This position is responsible for providing primary care to Cares Community Health patients while also supporting Care’s Chief Medical Officer.

Essential Functions:

  • Perform as an active member of the medical team by providing patient care.
  • Provide site-based oversight of medical services delivered by providers in the practice, which will include Physicians (MD/DO’s), Nurse Practitioners, and Physician Assistants.
  • Ensure that patient care/staff practice meet both safety/risk management standards and legal/regulatory requirements.

Essential Functions:

  • Analyzes pharmacy sales and revenue cycle performance data to identify trends, patterns, and root causes
  • Understand key revenue cycle metrics and performance indicators and provide reporting of operational and financial performance

Float Patient Services Representative

Cares Community Health, a non-profit Federally Qualified Health Clinic has an immediate opening for a full time Float Patient Services Representative. This position will support multiple departments when needed.

Essential Functions:

  • Promptly and courteously provides the information and services requested by patient.
  • Takes initiative to seek assistance from others; informs patient/clients of the need to do so; explains who is being called upon to help; and ensures that all patient/clients requests are addressed in a way which minimizes inconveniences of all parties.
  • Maintain strict patient/clients confidentiality at all times.
  • Verify and update patient demographic information and insurance information.
  • Communicate appointment delays effectively with patients and providers; apprise patients of wait times, delays, etc.
  • Verify patient insurance eligibility and co-payment. (HMO’s, PPO’s, Managed Care)

Payroll Administrator

Under the direction of the Finance Director, the Payroll Administrator’s primary responsibility is payroll and all related activities including processing and reporting

Essential Functions:

  • Review timesheets for completion and accuracy before processing
  • Process biweekly payroll utilizing the automated payroll system

EPIC Trainer & Specialist (Cadence & Prelude)

Cares Community Health, a non-profit Federally Qualified Health Center, has an immediate opening for a full time EPIC Trainer & Specialist (Cadence & Prelude). Responsible for the training and support of all internal users for patient registration, scheduling, and front desk operations using Epic’s Cadence & Prelude modules.

Essential Functions:

  • Works within Cares Community Health’s IT department under the direction of the IT Director and Epic Site Specialist conducting and documenting classroom & virtual training programs, administering tests, developing and maintaining training regimens for newly hired staff as well as additional trainings for existing staff as required for the Cadence and Prelude modules.
  • Provides second tier support for the Cadence and Prelude modules when a super user escalates an issue.
  • Leads the Practice Management Super User team, assess needs and areas of opportunity in this team, and brings them to the Epic Site Specialist as appropriate.
  • Maintains up-to-date expertise in the operation and application of Epic’s Cadence and Prelude modules and a working knowledge of indicators, standards, and processes related to these systems. Works with the Practice Manager to improve and streamline related processes.
  • Helps implement our continuous quality improvement program and Patient Centered Medical Home workflows as they relate to patient registration, scheduling, and front desk operations. Follows up after training to make sure workflows are implemented.

Nutrition and Diabetes Education Services Supervisor

Cares Community Health, a non-profit Federally Qualified Health Center, has an immediate opening for a full time Nutrition and Diabetes Education Services Supervisor. This position will supervise Nutrition Services including medical nutrition therapy in an outpatient primary care setting; and manage our Diabetes Self Management Education Program as we work towards national accreditation.

Essential Functions:

  • Oversees the planning, implementation and evaluation of Diabetes Self- Management Program and steps necessary to secure National Accreditation
  • Responsible for applying for and maintaining National AADE Accreditation
  • Provides diabetes self management education to patients in individual and group settings
  • Assists primary care providers consulting on care and treatment of diabetic patients and provides staff diabetes training and education as needed
  • Supervises registered dietitian nutritionist providing medical nutrition therapy and nutrition related classes and wellness campaigns
  • Assists in supervision of Sacramento State post graduate Dietetic interns and undergraduate community nutrition interns
  • Participants as part of organizational leadership team
  • Assists organization to meet Patient Centered Medical Home bench mark goals
  • Participates in clinical co-ordination with nursing and other medical team staff
  • Along with the Compliance Department, monitors and facilitates maintenance of staff qualifications (CE credits, licensures, and registrations) as necessary to maintain program certification

Qualifications

  • Minimum of 3-5 years experience as a registered dietitian and certified diabetes educator
  • Bi-Lingual Spanish speaking preferred

Compliance Analyst

Cares Community Health has an immediate opening for a full time Compliance Analyst. The Compliance Analyst monitors and audits activities for regulatory compliance, policy and procedure review, specialized research on compliance issues and activities contained in the Cares Community Health Compliance Program and Plan.

Essential Functions:

  • Manage billing and coding compliance projects that require research, data collection, and audit analysis of current Cares Community Health billing and coding processes and evaluation of applicable regulations and guidelines.
  • Complete auditing and monitoring activities, prioritize and provide supporting audit results and statistics, data analysis, research and other project results. Perform charts reviews and monitor activities for compliance. Prepare summary of resolved issues to Compliance Manager.
  • Review, recommend and implement changes to organizational forms, reports and organizational policies and procedures.
  • Manage multiple audit and document review projects assigned to achieve the scope, timing, and objectives of each assignment.
  • Report findings and recommendations to Compliance Manager. Submit suggestions in writing or oral presentations.
  • Regularly check Compliance Hotline, maintain a log of calls received and report calls to Compliance Manager for investigation.
  • Perform monthly exclusion checks. Report finding to Compliance Manager.

Education/Experience Required:

  • Bachelor’s Degree in Healthcare administration or related field required
  • Three (3) years experience working in business, professional or health care environment, including one year experience in compliance, internal audit, billing, coding or related field is required.
  • Health Care Compliance (CHC) or auditing certification preferred.
  • Experience with data analysis tools and techniques.
