Primary Care Physician

Cares Community Health is seeking a Primary Care Physician to see walk-in, urgent care and pre-scheduled patients. Site Medical Director

Cares Community Health has an immediate opening for a full time Site Medical Director. This position is responsible for providing primary care to Cares Community Health patients while also supporting Care’s Chief Medical Officer.

Essential Functions:

  • Perform as an active member of the medical team by providing patient care.
  • Provide site-based oversight of medical services delivered by providers in the practice, which will include Physicians (MD/DO’s), Nurse Practitioners, and Physician Assistants.

Junior Data Analyst

Cares Community Health, a non-profit Federally Qualified Health Center, has an immediate opening for a full time Jr. Data Analyst. The Jr. Data Analyst is responsible for the accurate and timely delivery of scheduled reports to the various department of Cares Community Health’s electronic medical records system.

Essential Functions:

  • Works under the direction of the Lead Data Engineer within Cares Community Health to develop and run basic to intermediate level queries to pull a wide variety of information from our data systems for use internally and externally using SQL Server, SSMS and/or Visual Studio.
  • Works to improve the presentation of the data with end users thru SQL Server Reporting Services (SSRS) and Excel.
  • Performs all necessary checks and reviews of reports to ensure the highest level of accuracy.

Qualifications

Senior Accountant

Cares Community Health has an immediate opening for a full time Senior Accountant. This position is responsible for reviewing monthly grant/contract invoicing to federal, non-federal and private grantors and ensures proper compliance with all GAAP, federal, OMB and CFR, grant/contract regulations, standards, and guidelines. Also, review the expenditures to make sure general ledger classifications are correct and in line with post award program budgets. This position is the lead in the department in making sure we are compliant and audit-ready at all times. Departmental policies and procedures are the responsibility of this position.

Essential Functions:

  • Provide oversight of the financial and record-keeping requirements for all Private, County, State and Federal Grants through working closely with accounting staff to ensure accurate financial reporting and procedural compliance on all grants.
  • Review grant invoices for final posting by Finance staff
  • Review reconciliation of Grant Invoices Account Receivable report
  • Responsible for time and effort reporting system, ensuring it is complete and in compliance with applicable regulations; maintains regular communications with applicable departments; then communicate with finance department for problem resolution.
  • Review the general ledger accounts that have grant revenue and expenditures for accuracy monthly prior to our close.
  • Review analyzed data from EHR for accuracy and to determine correct payer source for grant funding.
  • Must follow Federal, State and local grant regulations and have knowledge of these ever-changing regulations.

Education/Experience Required:

  • BS/BA in accounting or equivalent experience.
  • 3-5 years’ of accounting experience and working with federal grants is required

Float Patient Services Representative

Cares Community Health, a non-profit Federally Qualified Health Clinic has an immediate opening for a full time Float Patient Services Representative. This position will support multiple departments when needed.

Essential Functions:

  • Promptly and courteously provides the information and services requested by patient.
  • Takes initiative to seek assistance from others; informs patient/clients of the need to do so; explains who is being called upon to help; and ensures that all patient/clients requests are addressed in a way which minimizes inconveniences of all parties.

Registered Nurse

Cares Community Health has an immediate opening for a full time Registered Nurse. Under the direction and supervision of the Nursing Manager; the Registered Nurse is responsible for providing direct patient care to a diverse patient population, across the continuum of care.   This position may include duties of charge nurse as assigned by the Nurse Manager or Clinical Lead RN to facilitate continuity of clinic operations.

Essential Functions:

  • Complete patient’s initial assessment and adjust the patient treatment plan as appropriate.
  • Develop a plan of care that reflects an interdisciplinary approach.
  • Assess patient’s safety, comfort and self-care needs across the continuum of care.
  • Administer intravenous medications, skin tests, venapuncture, and Pentamidine treatments upon provider’s orders, according to standardized procedures.

Medical Scribe

Cares Community Health, a non-profit Federally Qualified Health Clinic has an immediate opening for a full time Medical Scribe. It will be the responsibility of the Medical Scribe to annotate any dictated or written information for the treatment of patient following all local, state, and federal guidelines for documentation into the electronic medical record. Medical Scribes will be responsible for clearly recording each patient’s symptoms, history, physical exam, and documenting the diagnosis as well as treatment plans. The Medical Scribe is not a direct patient care role.

Essential Functions:

  • Every Scribe must become certified by passing the Medical Scribe Certification and Aptitude Test (MSCAT) once they have worked 200 hours on the clinical floor.
  • The primary role of the Medical Scribe is to assist the clinician with documentation of each patient’s medical chart during his or her visit.
  • The Scribe accompanies the clinician into the patient examination area in order to transcribe a history and physical examination as given by the patient and clinician.
  • The Scribe, under the direction of the clinician, transcribes patient orders, including laboratory tests, radiology tests, referrals etc.
  • The Scribe documents any procedures performed by the clinician or nurses, on the electronic medical chart.

Education/Experience Required:

  • Medical Terminology coursework.
  • Basic computer proficiency in Windows environment.
  • Strong typing skills (45-50 WPM).
  • High school diploma or equivalent.
  • Three (3) years experience as a Medical Assistant (MA)

Chief Operating Officer

The Chief Operating Officer has responsibility for the administrative clinic operations of Cares Community Health under the direction of the Chief Executive Officer including development and achievement of fiscal and operational objectives, direction and implementation of initiatives and continuous quality assessment monitoring in conjunction with the functional department heads.

Essential Functions:

  • Under the guidance of the Chief Executive Officer, develops and implements operational plans leveraging current technologies to maximize cost efficiencies and productivity while promoting Cares Community Health as the primary care provider of choice in the Sacramento region.
  • Provides general supervision for the daily management and operation of Cares Community Health services, in order to achieve delivery of programs and services in a high quality and fiscally responsible manner, which is sensitive to the varied needs and backgrounds of Cares Community Health patients.
  • Ensures clinic is appropriately staffed during business hours for service provision, within context of established budget parameters. Recommends to the Chief Executive Officer establishment and/or deletion of positions.
  • Under direction of the Chief Executive Officer, develops and presents an annual operating budget. Operates clinic and related activities within established budget parameters.
  • Supports the Chief Executive, by providing information, analysis and reports to inform and support the Quality Management Committee, Finance Committee, Executive and Compliance Committee, Governance Committee and the Communication and the Advocacy and Fund Development Committees’ work.
  • Develops, implements and monitors systems designed to fully comply with contract and/or grant requirements as provided to Cares Community Health. Negotiates contract language when appropriate, seeking legal counsel and Chief Executive Officer approvals.
  • Reviews performance of Directors/Managers and others reporting directly to the Chief Operating Officer.
  • Provides oversight, supervision (jointly with the Chief Medical Officer and Chief Quality / Nursing Officer), and support for the Operations Director in carrying out administrative functions to support the care teams including reception staff, clinic clerical support, and scribes.
  • Provides oversight, supervision, and support for Support Services Director in carrying out functions of centralized services including call center, referrals management and peer advocates.
  • Provides oversight of the Director of Facilities in management of facilities projects, improvements, construction, and new building acquisitions.
  • Provides oversight of Health Information Services Supervisor in the ongoing management of health information exchange internal and external to the Cares Community Health centers.
  • Provide leadership and exercise team building and conflict resolution skills with the staff while developing a team that delivers superior patient services.
  • In collaboration with the Chief Medical Officer and Chief Quality / Nursing Officer, assesses functioning of the clinic and the quality, effectiveness and efficiency of operations to ensure best practice.
  • In collaboration with the functional area department heads, develops new programs and services for Cares Community Health, identifying grant-seeking and writing opportunities for initiation as appropriate. Directs and monitors new clinic programs to achieve established objectives.
  • In collaboration with the Chief Medical Officer and Chief Quality / Nursing Officer, develops, implements and monitors communications systems designed for safe and efficient operations and resolution of problems.
  • Participates in quality improvement activities including annual training in quality improvement.
  • Other responsibilities as assigned.

Under the direction of the Finance Director, the Payroll Administrator’s primary responsibility is payroll and all related activities including processing and reporting

Essential Functions:

  • Review timesheets for completion and accuracy before processing
  • Process biweekly payroll utilizing the automated payroll system

AOD Counselor

Under the direction of the Alcohol and Other Drug Program Supervisor, with guidance from the Behavior Health Manager, the Alcohol and Other Drug Program Counselor is responsible for providing alcohol and drug counseling and case management services and referrals to patients.

Essential Functions:

  • Complies with established department policies and procedures, objectives, quality assurance program, safety, environmental, and patient service standards.
  • Provide chemical dependency intervention and counseling to individuals, including significant others and family as requested by the patient.
  • Conduct patient counseling and assessments, using various accepted skills, tools and intervention methods, in conjunction with other providers where necessary, to develop plans for provision of services. Work with patient to develop agreements and maintain plans regarding goals and objectives for alcohol and drug dependency treatment.
  • Communicate with other providers within the organization to encourage coordination in patient care, including medical other social services needs. Communicate externally with agencies and other entities, providing referrals and additional support for services as needed.
  • Conducts all work within generally accepted standards and ethical guidelines of the social work or counseling professions. Seeks out guidance from BH Director/BH Manager/Clinical Supervisor/ Alcohol and Other Drug Program Coordinator where questions or concerns may arise.

Education/ Experience:

  • Bachelor’s Degree in Social Work/ related field or 4 years AOD experience required.
  • Current Alcohol and Other Drug Certification required. Licenses required for this position must be kept current, valid, and in good standing with the presiding entity.
  • CAADC II required
  • Spanish speaking preferred not required.
