For medical records information, contact Health Information Management at (916) 914-6305.  This line will be answered Monday through Friday 8:00am – 5:00pm.  After hours callers can leave a message.

You can also use the Cares Patient Portal, your medical home on the Web. With Patient Portal, you can connect with your doctor through a convenient, safe, and secure environment.
